Nancy Avitabile is the owner of Urban Eldercare, LLC, a Professional Geriatric Care Management practice based in Manhattan. She has been working with older adults for 20 years in the capacity of care manager, medical social worker and researcher. Ms. Avitabile is a LMSW and CMC. She holds a MSW, a BFA and BA from the University of Connecticut. In 2014, she became a Fellow of the Leadership Academy of the National Association of Professional Geriatric Care Managers (NAPGCM). As a recognized leader in care management in New York City, Ms. Avitabile has been invited to participate in many diverse New York City events such as the 2007 New York Academy of Medicine/Social Work Leadership panel on Long Term Care, the 2013 American Institute of Architects “Aging in Place” Charrette and the 2012 and 2014 Evelyn Frank Legal Resources Program fundraising benefit committee. She has been a program instructor at the Brookdale Center for Healthy Aging and Longevity of Hunter College and the Consortium of New York Geriatric Education Center.Ms. Avitabile is currently an active NAPGCM board member and the former New York Chapter President.
